BACS is seeing an Associate Director of Care Plan Supports overseeing our Enhanced Care Management (ECM) and Housing Community Supports programs.  These are foundational parts of the transformation of Medi-Cal focused on breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities. ECM and HCS introduce a better way to coordinate care for high-need members where they live. 

  • HCS provides care coordination in the realm of housing tenancy sustaining services, and housing navigation for those who have past experiences of homelessness.
  • ECM provides care coordination, healthcare navigation, and linkage to community resources to meet social needs such as housing, food, and other supports that promote improved health.

The Associate Director will manage direct service, administrative, fiscal, and quality improvement functions that promote the highest element of service provision for the direct care staff and thier clients.

 Responsibilities

  • Independently directs programs and ensures all contracts, laws, and regulations are met and works as part of the Operations Team to collectively run BACS programs.
  • Oversee multiple BACS signature community supports programs. 
  • Provide empowering, supportive and motivational field based supervision to managers and/or supervisors including hiring, training, and performance management.
  • Implement consumer-centered services that are needs-driven and strength-based, and culturally relevant.
  • Manage flow of referrals, intakes, and discharges of participants and ensures accessibility and welcoming of all people.
  • Maintain records and other administrative requirements of all programs; ensure compliance with documentation and charting requirements for all funding streams.
  • Never settle for status quo, implement continuous quality improvement standards and works with staff to implement systems for CQI.
  • Establish a harmonious and collaborative working relationship with the community and coordinate services as needed to highlight the value and the purpose of programming.
  • Participate in the development of new programs and services including grant writing, speeches, presentations and more; put on agency, professional and personal development trainings.
  • Assure that BACS services are provided in collaboration with all applicable funding streams and regulations; ensure contract compliance with all funders to include grant management, presentations, and reports.

Qualifications

  • BA required.
  • Minimum of 3 years experience managing revenue and expense for multiple Medi-Cal, CalAIM funded programs. 
  • Three years of progressive administrative, direct service, and leadership skills and experience in social services field serving high need individuals.
  • Demonstrated capacity leading diverse and multi-disciplinary teams toward a common goal required.
  • Valid California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage and the ability to drive for work.  DMV printout required.          

Compensation & Benefits

  • The annual salary starts at $130,000 and increases to $140,000 following successful completion of a 90-day introductory period, contingent upon satisfactory performance and achievement of all job-relatedd goals and requirements.
